Morning Presentation: "When One is Not Enough: Rebooting Satiation"
    Presenter: Brian Johnson, M.D.
    Panel Discussant: Brian Koehler, Ph.D.
Addiction has physical as well as psychological aspects. Working psychoanalytically with addicted patients requires one to understand the subcortical changes that drive their addicted behavior because these changes induce unconscious alterations in drives, defenses, and relatedness. Codependence countertransference can be a relational consequence for relatively unaware analysts confronted with the difficulties involved in treating such patients.
Brian Johnson, M.D., is director of addiction psychiatry and associate professor at CUNY Upstate Medical University. He is also co-chair of The Substance-Abusing Patient in Psychoanalysis and Psychotherapy, the annual workshop of the American Psychoanalytic Association. He is a long-term member of the American Academy of Addiction Psychiatry and a Distinguished Fellow of the American Psychiatric Association. Dr. Johnson's publications focus on the neuropsychoanalytic treatment of patients with addiction, prescription drug abuse, and the treatment of chronic pain. His e-book for children on addiction neuropsychoanalysis, Widespread Zombification in the 21st Century and the Wars of the Zombie Masters, is available at www.gegensatzpress.com/zombies.
Brian Koehler, Ph.D., is an adjunct associate professor at NYU's Postdoctoral Program in Psychotherapy and Psychoanalysis and Silver School of Social Work and at CUNY. He is faculty and supervisor at the Psychoanalytic Psychotherapy Study Center (PPSC). Dr. Koehler is a reviewer for the "International Journal of Psychoanalysis." He is also an associate editor of the new international journal "Psychosis: Psychological, Social and Integrative Approaches." As the U.S. chair and executive board member of the International Society for the Psychological Treatments of the Schizophrenias and Other Psychoses (ISPS), he has published many articles on the neuroscience and psychotherapy of psychoses. Dr. Koehler is in private practice in Manhattan and Westchester.

Afternoon Presentation: "Don't Stop 'Til You Get Enough Commonalities of Compulsive Sex and Drug Addiction"
    Speaker: Steven J. Lee, M.D.
    Panel Discussant: Judith Rustin, LCSW
Compulsive sex and drug addiction share neurobiological pathways. These common neuropathways explain similarities between sexual compulsion and chemical addiction and also help us understand changes in sexual behavior caused by several well-known addictive substances. Dr. Lee will discuss the clinical presentation of an addicted patient with compulsive sexual behavior, considering therapeutic and pharmacologic interventions.
Steven J. Lee, M.D., is an assistant clinical professor of psychiatry at Columbia University, where he also supervises physicians training in adult psychiatry and the treatment of addictive disorders. He is certified by the American Board of Medical Specialties in adult psychiatry, addiction psychiatry, and addiction medicine, and he is also a Fellow of both the American Society of Addiction Medicine and the American Psychiatric Association. He lectures worldwide on topics of addiction, has been interviewed on radio shows and documentaries, and has written a book entitled Overcoming Crystal Methamphetamine Addiction.
Judith Rustin, LCSW, is faculty, supervisor, and training analyst at the Institute for the Psychoanalytic Study of Subjectivity (IPSS) and the Psychoanalytic Psychotherapy Study Center (PPSC) in New York City. She is the author of various papers on both intersubjectivity theory and its integration with infant research. Since 1995 she has been studying, writing, and teaching on the synthesis of neuroscience and psychoanalysis. She is a co-author (with Beatrice Beebe, Steven Knoblauch, and Dorienne Sorter) of the book Forms of Intersubjectivity in Infant Research and Adult Treatment. Currently, she is working on combining the neuroscience of addiction with clinical psychoanalysis.

"Retrieving the Missing Pieces: A Cross-Cultural Approach to Memory Fragmentation"
    Speaker: Sarah Jenkins, M.C., LPC

Ancient cultures, across continents, have identified the importance of processing dissociated aspects of the self. With this in mind, and staying true to the eight-phase EMDR treatment model, a cross-cultural, shamanic approach to processing sensory-motor aspects of trauma is provided.
    Objectives:
    • Attendees will be able to identify the relationship between the fragmented aspects of the sensory-motor aspects of trauma, ego state therapy, and cross-cultural shamanic approaches to trauma treatment.
    • Attendees will be able to identify specific indicators, including incomplete client narratives, that unprocessed fragments of the sensory-motor aspects of trauma are impeding client processing during EMDR.
    • Attendees will gain an increased understanding of fragmentation and its impact on trauma processing.
Sarah Jenkins, M.C., LPC, is an EMDRIA-Certified EMDR therapist and Licensed Professional Counselor. Her private practice is located in Tempe, where she specializes in treating survivors of childhood abuse. Ms. Jenkins is also an adjunct professor for the Arizona State University and University of Phoenix. In addition, she has also served as a Clinical Supervisor for a government-funded EMDR trauma treatment program. Ms. Jenkins has lived in Europe and the United States and seeks to bring a cross-cultural perspective to her clinical work.

"Evaluation and Management of Pain Through the Prism of The Healing Response"
    Speaker: Michael W. Loes, M.D., M.D.(H.)

This talk addresses pain through the eyes of what is known as “The Healing Response,” which is the title of Dr. Loes’ book. The approach is primarily to empower patients' quest to obtain optimal health and teaches a practical path to do so by utilizing ten principles that lead to healing, i.e., increase physiological power, direct energy to heal, rid stagnation, respect body clock, understand solvent/solute issues, learn how cells communicate and how “Belief Creates Biology and, hence, Behavioral Health" (and why the converse is equally true). Others include protecting against oxidative stress, knowing what building blocks are needed to heal, and why embracing certain spiritual concepts, especially those involving relationships, are key.
    Objectives:
    1. To be able to define “The Healing Response.”
    2. To be able to list at least four of the ten principles and accompanying action mandates which empower The Healing Response.
    3. To knowledgeably explain what multidimensional/multidisciplinary treatment really means.
    4. To know how integrating the following therapies are most useful: EMDR, Somatic Experiencing®, Zero Balancing, Reiki, Acupuncture, and Biofeedback.

    "Eating Disorders and the Secret Multiple Diagnoses;
    Speaker: Ms. Jeannette Rojas

Eating disorders often co-exist along with trauma, depression and self harm behaviors. Although clinicians have the DSM-IV-TR as a tool, with clear descriptions of different criteria for the various mental disorders, there is some disagreement in cases of dual diagnosis, selecting what is the primary problem. This workshop will provide strategies and interventions to treat eating disorder patients who have a dual diagnosis such as eating disorder, depression, trauma, and self harm behaviors.
    Objectives:
    1. The participant will be able to identify the relationship between eating disorders and trauma, self harm behaviors, and depression and practical techniques to address and stop self harm behaviors.
    2. To provide practical techniques to help patients to express anger in a healthy way.
    3. The participant will learn practical tools, exercises to treat eating disorder patients who have depression, trauma or self harm behaviors.

"Theory and Efficacy of Wilderness Therapy "
    Speaker: Brad M. Reedy, Ph.D.

This presentation challenges the assumptions put forth in Maslow’s hierarchy of needs and asserts that family values, sense of self, and healthy skills for living can be gained while struggling for survival. Wilderness therapy uses the metaphor of primitive living, borrowing from Native-American living and rituals, to teach values and foster healthy ideas for living in the lives of adolescents and young adults struggling with addiction and other mental health challenges. The use of metaphor also helps to bypass resistance common in adolescents with treatment failures or low motivation for therapy.
    Objectives:
    1. To explore the underlying theory of wilderness therapy.
    2. To review research of efficacy of wilderness therapy, identifying features that add to efficacy.
    3. To review goals and desired outcomes for participation in wilderness therapy.
Brad M. Reedy, Ph.D., LMFT, is the Executive Director of Second Nature Therapeutic Programs. He directs the oversight of the Wilderness Programs in Utah, Georgia, and Oregon. Dr. Reedy creates and conducts online semi-weekly Webinars. He organizes and conducts National Parent Meetings and oversees the Therapeutic Boarding School. His Public Relations efforts include interface on nationally syndicated radio talk shows. In 2008 he was appointed to sit on the Utah Department of Child and Family Services Board by Utah Governor Huntsman to participate in the program oversight and contribute to legislation for departments in child protection and foster care services. Also in 2008 he was appointed to sit on the board of The National Association of Therapeutic Schools and Programs (NATSAP). Dr. Reedy holds a Ph.D. in Marriage and Family Therapy from Brigham Young University and a Master of Science in Marriage and Family Therapy from Loma Linda University.

    "“The Neurobiology and Treatment of Misbehavior: Addiction, Thrill-Seeking, and Dry-Drunk Behavior";
    Speaker: Robert R. Johnson, D.O.   Ph.D.

We don’t change, even when our life depends on it, but will risk everything for a brief high. In this presentation, Sierra Tucson’s Medical Director Robert Johnson, D.O. will enhance your understanding about the neurobiology of addiction, the nature of thrill-seeking, and faulty decision-making in drug and process addiction, and show how common neurobiological pathways involved in the thirst for adventure and meaning can be leveraged in the service of long-term recovery.
    Objectives:
    1. Understand the neurobiological pathways of addiction.
    2. Differentiate between lack of motivation for recovery versus brain-based factors that can derail momentum toward recovery goals.
    3. Articulate how normal brain development predisposes susceptible individuals to impulsive, unhealthy, and potentially addicting behaviors.
    4. Employ a tool that enables clinicians to recognize and resolve their countertransference to “acting out behaviors,” so they are able to engage patients with empathy, authenticity, and effectiveness.
    5. Understand how addictive behavior and thrill-seeking can hijack biological reward and reinforcement pathways and fuel hedonic treadmills that only worsen the problem.

    G. Robert R. Johnson, D.O., is the Medical Director of Sierra Tucson. He is a Board-Certified Psychiatrist and Addictionologist; his research background is in psychopharmacology, depression, immune functioning in psychiatric patients, and psychosomatic medicine. He is also a Certified Executive Coach who has worked with executives at Sony, O'Neil Sports, Rockwell, and the USOC, among other organizations.

Professionals are invited to attend the March 12 Grand Rounds presentation at Sierra Tucson.
    1.5 CE Credits Available!

    Schedule:

    Friday, March 12, 2010
    1:00 - 2:30 p.m.

    Location:
    Sierra Tucson
39580 S. Lago del Oro Parkway
    Tucson, AZ 85739

"Neurofeedback and Brain Mapping: The Science and Clinical Application"
    Presenters: Steven M. Orenstein, LMFT; Curtis T. Cripe, Ph.D.;
    and Terry V. Eagan, M.D.

Quantitative Electroencephalography (QEEG) or "Brain Mapping" is an FDA-approved, evidence-based diagnostic tool that measures brain function and processing ability, including speed of processing and emotional stability.

QEEG bio-markers have demonstrated predictive abilities for several psychiatric conditions including depression, learning disabilities, attention-hyperactivity disorders, anxiety, and brain-based addiction drives including relapse prediction probabilities. The presentation will demonstrate how the QEEG is essential in the developing protocols for neurofeedback training.
We will compare and contrast the different aspects of neurofeedback approaches to treatment and present several actual addiction case studies. Each case presentation will highlight QEEG bio-markers, including physiological underpinnings and expected neurobehavioral expression findings.

    Learning Objectives:
At the end of the presentation, participants should be able to:
    1. Understand the development of neurofeedback and its role in treating neuropsychiatric and addictive disorders.
    2. Describe what a QEEG ("Brain Map") is and how it can help inform neurofeedback protocol selection and overall treatment planning.
    3. Differentiate between the types of neurofeedback protocols that (a) reduce excessive brain activation and build self-regulatory capacity from those that (b) increase brain activation in specific regions and thus enhance cognitive performance.
Steven Orenstein, LMFT, served as Director of Addiction Medicine Services at Cedars-Sinai Medical Center in Los Angeles, CA, where he developed effective and cutting-edge dual diagnosis and chemical dependency treatment programs. He has over 30 years of experience as a psychotherapist, interventionist, and administrator, specializing in the treatment of co-occurring disorders. Utilizing the latest evidence-based science available and FDA-approved tools, Mr. Orenstein utilizes technology ("brain-mapping") that assesses over 293 dimensions of brain activity and cognitive abilities relating to learning, concentration, major depression, anxiety disorders, emotional trauma, and memory.
Curtis T. Cripe, Ph.D., is Director and Founder of Crossroads Institute. He holds a Ph.D. in Research Psychology; a Masters in Behavioral Medicine; a Masters in Neurodevelopment, Neurotherapy; and a Diplomate in Peak Performance.
Terry V. Eagan, M.D., is a psychiatrist and serves as Medical Director of Moonview Sanctuary. A graduate of the University of Tennessee College of Medicine in 1987, Dr. Eagan completed his medical training in psychiatry at the Los Angeles County-University of Southern California. After serving as chief resident in psychiatry at USC from 1990 to 1992, Dr. Eagan developed a highly successful private practice in Beverly Hills, CA. During his 16-year professional career, he has served in the capacity of Medical Director for numerous treatment centers such as Sherman Oaks Medical Center, Eating Disorder Center of California, Milestones Ranch Malibu, and Bridges to Recovery.
