Sierra Tucson Launches New Continuing Care Program

TUCSON, Ariz. – November 9, 2015 – Sierra Tucson, a leading residential treatment center in Tucson, Ariz., announced today the launch of its new continuing care program, Connect365. For one full year following treatment completion, participants will receive personalized support from an on-site Recovery Coach, continued encouragement from a support team, and access to an interactive platform via mobile device or computer.

“At Sierra Tucson, we are committed to partnering with patients on their journey of recovery. We want them to feel supported as they transition from residential treatment to the demands of everyday living. Connect365 will serve as a vehicle to encourage and empower them at no additional cost,” said Chief Clinical Officer Jaime Vinck, MC, LPC, NCC.

The Connect365 Program provides an efficient, user-friendly solution for establishing accountability and monitoring patients’ progress post treatment. Secure messaging, programmed alerts, daily assessments, and extensive reporting are just a few of the HIPAA-compliant platform capabilities. Ongoing communication with the participant’s Recovery Coach via phone and secure web chat helps guide him or her on the road to recovery. And a patient-identified/approved support team can offer positive reinforcement when appropriate.

Sierra Tucson understands that the first year of recovery sets the stage for long-term success. Through individualized support and relapse-prevention assistance, Connect365 fosters healthy recovery by keeping the participant connected and engaged. “Patient advocacy is at the heart of Sierra Tucson’s patient-centered approach. Connect365 allows for collaboration and accountability following discharge, so we can create meaningful, effective, and measureable change,” said Vinck.
